How do I do html form file uploads?

Following the PHP Documentation on file uploads is all you need to do. An example is contained in the first annotation below.

If you wanted to let anyone upload files to your account, this (or some other web script) is the only way to do it since we do not allow Anonymous FTP.

For file uploads larger than 8MB, you must have these settings in your
/etc/php.ini file:

 upload_max_filesize  = (some number greater than 8 million)
 post_max_size  = (some number greater than upload_max_filesize)
 memory_limit = (some number greater than the 2 settings above)

If the "post_max_size" configuration setting is not in your /etc/php.ini
you need to add it.

Note that you should never allow anonymous visitors to upload files to
your account, particularly to a location that is web-accessible. Doing
so creates an opportunity for hackers to install and run malicious 
software in your account. Therefore the destination of your uploads
should be inaccessible to web browsers or password protected.

/logs/php_error_log or /logs/php5_error_log
